This is a sign that an auto-revive which was supposed to activate failed. Keep a manual Self Reviving handy in solo and don't depend solely on this perk.  Elemental Pop & Random Deaths: As stated, certain Pack-a Punch or Elemental Pop effects can sabotage a precision challenge.
